Transportation:
Car Transportation by a parent is the preferred transportation. We may provide transportation on a limited basis. You will be notified if space is available.
Medical Authorization:
I (parent/guardian/self) hereby authorize Calvary Baptist Church leaders to administer first aid and to obtain and consent to on my behalf any emergency first aid or medical care by any physician or hospital for the participant (named above). I agree to abide and be bound by such decisions and consents as if made by me. I further authorize any physician, hospital or medical attendant to receive full and complete medical reports or information deemed necessary with respect to the treatment of the participant listed above. Execution of this document shall operate as an authorization for such person(s) to receive any medical information which they require. I will also hold harmless and release from all liabilities Calvary Baptist Church, Cleburne, Texas and its staff members, and workers from any injuries and/or illnesses occurring to the participant names above during any official activity including transportation as requested/noted below.
Photo Authorization:
I/We the undersigned hereby consent to the use of the particant's (named above) photograph/video image/digital in any website, brochure, publication, or video produced by Calvary Baptist Church. I/We understand that this is on a voluntary basis and I/We will not be compensated for the use of my/our images.
Payment for 2026-27 On Track Wednesday Program
Calvary Baptist Church is committed to offering a strong, Christ-centered program for our Wednesday On Track program. Our church family is wonderfully generous, and we work hard to steward every gift to God’s glory.
To help cover the cost of meals, games, awards, books, crafts, and supplies, we are asking for a $25 participation fee per student for the 2026–27 On Track Wednesday ministry year.
We also understand that some families may be facing special circumstances. Sponsorships are available — no questions asked. Others may feel led to give above the fee to help another child attend.
We are grateful for every student, every volunteer, and every gift. The form below allows you to pay as you are able.
